Ontario Health Teams

The response has been extremely positive! Over 150 self-assessment submissions to become an Ontario Health Team were received from across the province, with over 1000 different providers and organizations signing up. 

Self-assessments represent providers from across the continuum of health care and beyond. This demonstrates a broad commitment to integrating care and to enabling patients, families, communities and providers to better work together to improve patient care transitions, experience and outcomes.

Updated Timelines

Because of the incredible interest we have extended our timelines. Please see below.

Deadline to submit self-assessments May 15, 2019
Selected groups will be invited to submit a full application Early July 2019
Deadline to submit full applications September 2019
Announce Ontario Health Team candidates Fall 2019
Deadline for Second Round of self-assessments Fall 2019

What’s happening now

All submissions are being fully reviewed. The ministry is reaching out to the primary contacts identified in the self-assessments this week to confirm next steps in the review process.

This is an exciting time for health care in Ontario. If you have already submitted a self-assessment — thank you! The ministry will be in touch about next steps.

If you have not submitted a self-assessment, we encourage you to become familiar with the Guidance document and form and to connect with partners in your region. The date for the next call for self-assessments will be announced in the coming weeks.

The Ontario Health Teams web page is the best place for you to access the most up to date information, and resources.

The Connecting Care Act, 2019 comes into effect

On June 6, the provisions of The Connecting Care Act, 2019 came into effect. This new health statute was introduced under Bill 74, The People’s Health Care Act, 2019.

The Connecting Care Act, 2019 lays out the foundation for the continued implementation of Ontario’s phased strategy to transform and strengthen the public health care system. Several other Acts were amended so they would align with The Connecting Care Act.
