First Cohort of 24 Ontario Health Teams

This is an exciting time in health care for Ontario as the first cohort of 24 Ontario Health Teams have been announced. Congratulations to all the teams for your commitment to improving care for Ontarians.

These 24 teams will implement a new model of organizing and delivering health care that better connects patients and providers in their communities to improve patient outcomes. Through an Ontario Health Team, patients will experience easier transitions from one provider to another, including, for example, between hospitals and home care providers, with one patient story, one patient record and one care plan. The first 24 are:

Ontario Health Teams will play an essential role in delivering on the government’s commitment to end hallway health care and building a connected and sustainable public health care system centred around the needs of patients.

This is just the beginning for Ontario Health Teams. The ministry will continue to work closely with all teams, regardless of where they are in the readiness journey, towards becoming an Ontario Health Team. New teams will be announced on an ongoing basis as each becomes ready to deliver the new model of care that will better support patients throughout their entire health care journey.

All teams are encouraged to check the Ontario Health Team Central Program of Supports regularly for up-to-date resources, tools and news. The Digital Health Playbook has recently been updated based on feedback from the teams and stakeholders.
