New Family Health Teams
The new Family Health Teams will improve access to health care across Ontario. These new teams will also help to reduce the number of Ontarians who do not have a family doctor, and improve chronic disease prevention and management.
Wave 5 Family Health Teams
The Wave 5 application process for 30 new FHTs was announced on May 6, 2010 and closed on June 25, 2010. The announcement to award the successful applicants occurred on August 24, 2010.
