2002 MALPRACTICE PREMIUM REIMBURSEMENT PROGRAM – OPTION C 

Physicians are reminded that their application for Reimbursement of Malpractice Fees for the year 2002, (Option C only), must be received by the Ministry of Health and Long-Term Care no later than June 30, 2003.

You must return the completed application and your 2002 Canadian Medical Protective Association (CMPA) Acknowledgement to the Ministry of Health and Long-Term Care.

If covered by an insurer other than the CMPA, a receipt demonstrating proof of payment for the year 2002 must be enclosed.
