Where can I get more information on the privacy statement?

How does ColonCancerCheck use my personal health information?

ColonCancerCheck compiles personal health information in the Colorectal Cancer Screening Registry in order to:

Identify individuals who should be screened for colorectal cancer;


How can I withdraw from the ColonCancerCheck program?

At any time, you may ask ColonCancerCheck not to contact you. If you choose to do this, you will still be able to do an FOBT if you wish. After that:
ColonCancerCheck will not invite you to do colorectal cancer screening or remind you to do your FOBT again.

If you do an FOBT, ColonCancercheck will not send you your results. You may get them from your family doctor or nurse practitioner. If you do not have a family doctor or nurse practitioner, we will only send you your results if you ask us for them directly.

What do I do if I dont wish to be contacted by ColonCancerCheck?
