Re:   Reminder – Streamlined Enrolment and Consent Process

Further to INFOBulletin #11056 issued in 2013 – this is a reminder that effective February 1, 2013 the ministry introduced a process whereby physicians are no longer required to batch and submit paper copies of the "Patient Enrolment and Consent to Release Personal Health Information" (E/C) forms to the ministry.

Patients are formally enrolled and enrolment records are updated through the submission of the Per Patient Rostering Fee (PPRF) Code (Q201A).

To ensure your patients have been successfully enrolled, please review your "Roster and Capitation Payment Reconciliation Report" each month to ensure the patient appears. If your patient(s) do not appear on the report, verify your claims error reports and make any corrections needed before resubmitting. If there is no reject for the patient's Q200A/Q202A then submit the appropriate fee code.

Patients and physicians must continue to complete the E/C form and retain a copy for their files but paper submissions to the ministry are no longer required.

Any forms submitted to the ministry will not be processed or returned to your office and will be securely destroyed effective immediately.
