Quality Improvement Plans

System-wide quality improvement is the vision that the Ministry of Health and Long-Term Care (ministry) provides for all health care sectors, using the Quality Improvement Plan (QIP) as an enabler to support this goal.

Most health care sectors are familiar with developing Quality Improvement Plans (QIPs) as a way to express quality goals and targets for their organizations. All sectors are at a different starting point when it comes to developing QIPs. In providing a common set of guidance materials and supports, organizations will be facilitated in their efforts to develop a QIP under the umbrella of this common provincial vision to harmonize efforts to improve quality of care across the health care system.

The set of guidance materials listed below is intended to support organizations in the development of a QIP.

Sector-specific QIP templates are available to assist in the development and submission of your QIP. Organizations should access their sector-specific QIP template through the Health Quality Ontario QIP Navigator. The Navigator is HQO's online platform that allows organizations to develop and submit their QIPs annually.

QIPs are to be submitted to Health Quality Ontario (HQO) by April 1 of every year.
