Guidelines for Community Care Access Centres (CCACs) and Community Support Service (CSS) Agencies

The Guideline for Collaborative Home and Community-Based Care Coordination outlines the legislative, regulatory and policy standards that all agencies approved under the Home Care and Community Services Act, 1994 to provide community servicesmust follow, as they work together to coordinate and optimize home and community care for older adults with frailty or long-term chronic conditions to support them in living safely and independently in the community.

The Guideline on the Delivery of Personal Support Services outlines common standards that all agencies approved to provide in-home personal support services must follow. This guideline does not apply to:

  • Personal support services provided under the Attendant Outreach Services – Policy Guidelines and Operational Standards (1996), the Self-Managed Attendant Services in Ontario - Direct Funding Pilot Project - Policy Guidelines (1994) or the Long-Term Care Supportive Housing Policy (1994); and
  • Approved Agencies that provide personal support services to children or Approved Agencies that are not CCACs that provide personal support services to people with Acquired Brain Injuries.
