Transitions:

On June 7, 2017, Toronto Central CCAC and Central CCAC successfully transferred to their respective LHINs.  Here is what the CEOs said about their transition:

Members of the integrated executive team and the Board Chair welcomed colleagues to the new organization and recognized achieving this key milestone in our journey together. Spotlight On: Achieving Excellence Together Conference

This year marked the 18th year of the “Achieving Excellence Together” conference. This conference
brings together health and home care professionals, leaders, , care coordinators, service providers, and front-line staff to share innovative ideas and approaches and best practices. This two-day conference provides an opportunity to demonstrate the complex and multifaceted home and community care sector. Keynote speakers Dr. Susan Frampton and Dr. Danielle Martin both highlighted the importance of a patient-centred approach to care and the opportunities brought by system changes to transform our health care system to be able to serve Ontarians better.

With the CCAC to LHIN transitions nearing completion, this conference sets the stage for the next phase of transformation. With sessions ranging from collective impact models to e-notifications to wound care standards, there was inspiration for the many ways LHINs can improve care in the home care sector and beyond.

In his remarks, the Minister of Health and Long-Term Care thanked the conference participants for their leadership in putting patients first and acknowledged the dedication and humanity in the home and community care sector. He also shared his aspirations and commitments to universal health care including pharmacare coverage for those 24 and under.

As part of this annual conference, the Awards for Excellence recognize outstanding achievement and commitment in home and community care. There were six categories including the Citizenship Award, the Leadership Award, the Staff Team Award, the Individual Staff Award, the Sector Innovation Award and the System Partnership Award.
